[Dev] Server debugging

The behavior of the ­debug flag has been changed to not suspend pending
startup.  If suspend-before-start behavior is desired, use the new ­debugw
flag.  This is an acknowledgement that the majority of issues requiring
debugging occuring within the server are no longer startup-time issues, but
rather operation-time issues.
